From Disclosure to Practice: Free AI Informed Consent Template for School Psychologists


In May 2024, we asked a simple question: Should school psychologists disclose the use of AI? It became the most-viewed post in School Psych AI history.


The School Psych AI Ethics Council held a spirited discussion on the topic's complexities, and almost two years later, those findings remain valid. 


The recommendation?


Yes, disclosure is best practice, but where you disclose matters just as much. The Council suggested disclosure should happen during the informed consent process.


Since then, the most common follow-up question has been just as practical: What should that actually look like?


Importantly, this topic should not take attention away from the child. The focus should always be on creating a holistic picture of the child to help them find their version of success. These two templates will help you implement disclosure during the informed consent process, in a way that is transparent, clear, and keeps the main thing the main thing.


Our team has created two templates available for public use. Both are FERPA-focused, practitioner-written, and designed to be adapted to your context.

The first template is the full informed consent form in case you are starting from scratch. It is a ready-to-use, standalone informed consent form covering AI use, data protection, and parent rights.
